Home health services provided through Healthstar include:
SKILLED NURSING CARE
The nurse provides skilled care as ordered by the physician. These services include:
Observation and evaluation of the patient’s condition to prevent re-admittance to the hospital.
Teaching and training regarding the patient’s care and particular illness.
Nursing procedures: |
- Wound care
- Injections
- IV therapy
- Chemotherapy
- Tube feedings
- Catheter care

- Drawing blood
- Colostomy care
- Dressing changes
- Tracheostomy care
- Set up and supervise medications
|
- A variety of other services as determined by the patient’s physician.
- A registered nurse coordinates all care to make sure that the person’s total needs are met.
- If your physician has ordered home care for you, your nurse will call you the morning after your discharge
from the hospital.

HOME HEALTH AIDE/HOMEMAKER
Supervised by a registered nurse, the home health aide/homemaker performs such services as:
- Personal care (bathing, nail cutting, and grooming assistance).
- Simple meal preparation.
- Light housekeeping duties
- Assistance with routine exercises.
- Other daily living activities as directed by the registered nurse.

OCCUPATIONAL THERAPY
An occupational therapist is available to work with the homebound patient to provide:
- Evaluation of the patient’s ability to perform daily living activities.
- Teaching and retraining of daily living activities such as bathing, cooking, dressing, etc.

SPEECH THERAPY
A speech therapist is available to provide assistance with:
- Evaluation and treatment of communication disorders relating to speech, language and hearing.
- Carrying out a rehabilitative program to help the patient regain lost or diminished communication abilities.

PHYSICAL THERAPY
A physical therapist is available to provide assistance with:
- Evaluation of the patient’s muscle strength, sensation, nerve function and specific functional abilities.
- Performance of exercises and treatment programs to improve the patient’s ability to walk or increase the ability to use an arm or leg affected by disease or injury.

MEDICATION MANAGEMENT PROGRAM
A nurse will come into your home and set up your medication in an easy-to-use organizer. Information
sheets will be provided to assist you in understanding your medication schedule. The nurse will answer
any questions you have regarding your medications.

LIFELINE® PERSONAL EMERGENCY RESPONSE SYSTEM
A personal emergency response system gives you the confidence and freedom to live on your own. There
is a caring Lifeline monitor available 24 hours a day, 7 days a week to speak with you and send the help
you need.

FOOT CARE
Healthy foot care includes soaking, cutting, filing and applying lotion to feet. This service can be set
up by calling the Healthstar office to make an appointment. Homebound patients may call the Healthstar
office to make arrangements for the visiting nurse to see them in their home or apartment.
